My own personal experience only comes from time in RotorWay Exec 90 VH-ROT as well as initial flight tests and Type Ratings for owners in their new built machines. I still remember the machine which had the control cyclic cables crossed!

I cannot emphasis enough the importance of good pilot training - especially the ability to be continually mentally calculating how the ambient conditions (DA) v operating weight effect this helicopter's performance and available engine power. I normally operated the machine in ambient conditions equivalent to a DA of about 2,000 ft AMSL and regularly had full throttle power and not enough power for anything but a running take off.
